It includes a New Customer Checklist, you and/or your company's info, deal memo, B&H Guarantee of Service, Credit Application, Credit Card Authorization form, and a California Resale Certificate. The file itself is a .xlsx file and can be opened with MS Excel (2007 and later) and iWork's "Numbers".
